How to download, fill out, and save Muirfield Design Control Committee forms
A PDF reader is needed to use these forms. We suggestion using Adobe Reader X or later because Adobe enables you to fill out forms and save them on your computer. Other PDF programs may not let you save data or fill in the form on your computer, but will let you open and print the form. You can get Adobe Reader here for free if it is not already installed. Please pay attention when downloading. Adobe may try to add additional software that you may or may not want.
Sometimes forms will not open in the browser because of security settings or other restrictions. In this case, please follow the download directions below.
These forms can be filled out on your computer if you use Adobe Reader X or later. The form must be downloaded first. To download any form:
1. Find the form you want.
Once the form is open in Adobe Reader, you can fill it out on your computer. Fields are usually highlighted and you can type directly into any of those fields. Click on a field to type in it or use the Tab key to move from field to field.
For the signature block(s), Adobe is able to create a digital signature for you if you don't already have one. If you have difficulties with that field, you have two options:
1. If you print the form, you can sign it by hand.
Once the form is complete, click the Save button. You can then print it or email it to us at [email protected]. Please remember to include all of the information the form requests when submitting it.
Special note for Mac UsersIf you plan to email your completed form to us, do not use Mac Preview to fill out the form. Data entered in Mac Preview will not save in a compatible format and we will receive a blank form from you. Instead, please save the form to your computer and fill it out using Adobe Reader X or later, then email it to us.
If you intend to print the form, using Mac Preview works fine.
